Book a Facility Appointment for Registered Residents

This form is for submitting Virtual Care Review requests for existing residents registered with Aged Care GP (ACGP) only. Requests submitted during business hours (9:00 am to 5:00 pm, Monday to Friday) will be triaged by a nurse, who will arrange a virtual care consultation with a doctor. Requests submitted outside these hours will be reviewed on the next business day.

Please note that, due to time constraints, some reviews may need to be rescheduled to the next available appointment with a Virtual Care Doctor. If the situation is urgent, please do not wait for the rescheduled review. Seek further medical advice from a locum service, Virtual ED, or call an ambulance if needed.
